4. Describe and provide links to the stolen content

5. Detail why you are reporting this content and include additional information

6. Sign and submit your report

To complete your DMCA takedown on Facebook, review the provided declaration statement and sign the form, confirming that you are making the report in good faith and have ownership of the content.
